Applicants will have to be present in person, irrespective of their age, for submission of applications, except in the case of application in respect of new born children.
Please ensure that the application is filled in correctly and is complete in all respects and that clear copies all the required documents are attached. Incomplete applications will not be accepted.
Please bring original as well as clear copies of all documents. Copies of all the documents should be self-attested by the applicants. In case of minor children, copies of all the documents should be self-attested by one of the parents. Original documents will be returned immediately after verification.

REQUIREMENTS FOR APPLYING FOR PASSPORTS IN CASE OF ADULTS
1. APPLY ON-LINE at http://passport.gov.in/nri/OnlineRegistration.jsp?pocode and take a print out of the Passport application form. Please paste two recent colour photographs of the size of 2 inch x 2 inch with light back ground, one on the first page where space is provided and the second one on the blank space on the third page of the application. The photograph should have a frontal view, showing both the ears. The eyes must be open and no hair should obscure the face. Prescription glasses if worn should be clear and thin framed and should not have flash reflection or obscure the eyes. Head coverings, hair, head-dress or facial ornaments should not obscure the face. Head should face the camera directly and should not tilt or turned (portrait style). Head covering is not permitted except for religious reasons, but the facial features from bottom of chin to top of forehead and both edges of the face, with both ears must be clearly visible. REQUIREMENTS FOR APPLYING FOR PASSPORT FOR ENDORSEMENT OF SPOUSE NAME

REQUIREMENTS FOR APPLYING FOR PASSPORT IN CASE THE APPLICANT IS
ON FAMILY VISA

Clear copies of the (a) passports, (b) Permesso D’Soggiorno and (c) Carta D’Identita of the relative (father/mother/husband/son, etc.)on whose family visa the applicant arrived, and there should be documentary proof to substantiate the relationship, such as Marriage Certificate, Birth Certificate, etc. If the validity of the Soggiorno has already expired or Soggiorno applied for, a clear copy of the Receipt for Renewal of the same should be attached(self-attested). In case, the relative (father/mother/husband/son, etc.) is holder of OCI card, clear copies of his/her OCI card, Italian passport and Carta Identita should also be attached.
8. In case of any change in personal particulars (name, date of birth, place of birth, parents’ name, permanent address, spouse’s name, spelling mistake in any personal particulars, etc.), documentary proof for the same should be submitted.
REQUIREMENTS FOR APPLYING FOR PASSPORTS IN RESPECT OF CHILDREN
BELOW THE AGE OF 18 YEARS,

3. Clear copies of the relevant pages of the passport(self-attested by either of the parents)
4. Clear copy of the valid Permesso Di Soggiorno(self-attested by either of the parents). If the validity of the Soggiorno has already expired, a clear copy of the Receipt for Renewal of the same should be attached.
5. Clear copy of the Address proof(self-attested by either of the parents) ( Carta D’Identita or any other proof like any Utility bill-electricity/water/fuel bill or rent agreement, Ospity along with translation in English).
6. Clear copies of the relevant pages of the passports of both the parents(self-attested).
7. Clear copies of the Permesso Di Soggiorno of both the parents (self-attested)
8. Clear copies of Address proof ( Carta D’Identita or any other proof like any Utility bill-electricity/water/fuel bill or rent agreement, Ospity along with translation in English) of parents (self-attested)
9. In case one of the parents is away from the country of domicile, the other parent can give an Affidavit stating the reason for not taking consent of the parent, who is away from Italy.
10. In case of any change in personal particulars of parents (name, date of birth, place of birth, parents’ name, permanent address, spouse’s name, spelling mistake in any personal particulars, etc.), documentary proof for the same should be submitted.
REQUIREMENTS FOR APPLYING FOR PASSPORTS FOR NEW BORN CHILDREN
1. Register the Birth, on-line at : http://indiancitizenshiponline.nic.in/ic_form_public.aspx, within one year of the birth of the child. Upload the photograph of the child, signatures of both the parents, copies of the passports of both the parents, marriage certificate of the parents and birth certificate of the child (in English also).

2. If the birth of the child is not registered within one year of the birth, the reason for delay in registering the birth, in writing, duly signed by both the parents, is required. If the birth of the child is not registered within one year of the birth, the process will require longer time.
3. Take a print out of the completed form and submit along with the passport application form.
4. Parents should append their signatures (signature should match with that in their Passports) at two places on the second page of the Birth Registration application (where it is written Signature of parents).

Please note that since a new born baby cannot sign, his/her thumb impression (left hand in case of male child and right hand in case of female child) should be appended on the first page of the passport application form (space provided).
6. Both parents should append their signatures (signature should match with that in their passports) on Page No.2 of the application (Under Sl. No.26..DECLARATION) and also on the third page of the application form.
7. Copy of the birth certificate issued by Comune along with its English translation (self attested). The birth certificate should have the names of both the parents and date and place of birth.
8. Clear copy of valid Permesso Di Soggiorno of the child, if issued(self-attested by one of the parents)
9. Clear copies of all the relevant pages of the passports of both the parents(self-attested)
10. Clear copies of Permesso D’Soggiorno of both the parents( self-attested). If the validity of the Soggiorno has already expired, a clear copy of the Receipt for Renewal of the same should be attached(self-attested).
11. Clear copy of the Address proof(self-attested)( Carta D’Identita or any other proof like any Utility bill-electricity/water/fuel bill or rent agreement, Ospity and its translation in English) of the parents.
12. Copy of the Marriage Certificate(self-attested) of the parents if their names are not endorsed on each other’s passport.
13. In case of any change in personal particulars of parents (name, date of birth, place of birth, parents’ name, permanent address, spouse’s name, spelling mistake in any personal particulars, etc.), documentary proof for the same should be submitted.
OCI
REQUIREMENTS FOR RENUNCIATION OF INDIAN CITIZENSHIP (CANCELLATION OF INDIAN PASSPORT) CONSEQUENT ON ACQUIRING OF FOREIGN NATIONALITY AND FOR APPLYING FOR OCI CARD
Please bring original as well as clear copies of all documents, duly self-attested. Original documents will be returned immediately after verification.

INDIAN CITIZENS, WHO HAVE TAKEN FOREIGN CITIZENSHIP OR INDIAN CITIZENS BORN TO INDIAN PARENTS OR INDIAN ORIGIN PARENTS IN A FOREIGN COUNTRY OR THEIR GRANDPARENTS OR GREAT GRANDPARENTS WERE INDIANS OR INDIAN ORIGINS AND EVEN IF THEY HAVE NEVER HELD INDIAN PASSPORT ARE BASICALLY INDIAN ORIGIN ONLY. THEY MAY PLEASE APPLY FOR OCI CARD UNDER NORMAL CATEGORY.
FOREIGN NATIONALS, WHO ARE ELIGIBLE FOR OCI CARD BY VIRTUE OF BEING MARRIED TO INDIANS OR INDIAN ORIGINS MAY PLEASE APPLY UNDER "FOREIGN SPOUSE" CATEGORY. PLEASE NOTE THAT THE MARRIAGE SHOULD HAVE BEEN EXISTING FOR A MINIMUM PERIOD OF TWO YEARS.
Please submit the application with documents, keeping them in the following order:
FRESH APPLICATION FOR OCI CARD IN CASE OF ADULTS
1. APPLY ON-LINE at https://passport.gov.in/oci/capchaAction and upload the photograph and signature of the applicant.
2. Please upload the photograph, signature, copy of the Foreign Passport (one page where the photo, name, date of birth, etc appear), copy of the Indian passport (first page and last page) or the proof of being an Indian origin and marriage certificate, if the Indian passport does not have the spouse name endorsed in it. If the marriage was solemnized in Comune, the marriage certificate should first be apostilled from Prefutura and translation in English is mandatory to be uploaded. In case the marriage was solemnized in any other country, the marriage certificate should be attested from the concerned Indian High Commission/Embassy/Consulate.

7. Copy of the Marriage Certificate – self-attested (if married) if spouse name is not endorsed on their Indian passports. In case the marriage was solemnized in Comune in Italy, the marriage certificate should be apostilled from Prefutura and translation in English is mandatory. In case the marriage was solemnized in any other country, the marriage certificate, in English, should be attested from the concerned Indian High Commission/Embassy/Consulate.
8. In case any foreign national applying for OCI card by virtue of being married to an Indian national or an Indian origin : The marriage should have been solemnized at least two years before the date of submission of application for OCI card and the marital relationship is still existing. Marriage certificate and copy (self-attested) should be submitted. If marriage was solemnized in Comune, the same should be apostilled/attested by Prefuttura and original as well as clear copy of the same along with translation in English should be submitted. Further, the applicant and the spouse should give a joint affidavit stating that their marriage is still existing and that they have not sought/obtained divorce.
9. Copy of DECRETO regarding granting of citizenship issued by the concerned Foreign Government along with translation in English.
10. In case the Indian passport has already been cancelled earlier, copies of the relevant pages of passport where the photo/name/date of birth, etc. appear, page where the names of parents and address appear and the page where the Indian Consulate’s stamp appears with CONSEQUENT ON ACQUIRING OF FOREIGN NATIONALITY, THIS PASSPORT IS CANCELLED AND RETURNED TO THE HOLDER or the Renunciation Certificate-self-attested.
11. Application for cancellation of Indian Passport. One photograph to be affixed on the application form.
12. Copies of all the pages of the Indian passport (including blank pages) self-attested on all the pages.
13. In case the Indian passport has already been cancelled earlier, copies of the relevant pages of passport where the photo/name/date of birth, etc. appear, page where the names of parents and address appear and the page where the Indian Consulate’s stamp appears with CONSEQUENT ON ACQUIRING OF FOREIGN NATIONALITY, THIS PASSPORT IS CANCELLED AND RETURNED TO THE HOLDER or the Renunciation Certificate-self-attested.
IN THE CASE OF THOSE WHO ARE ALREADY HOLDER OF PIO CARD AND WANTS TO CONVERT IT INTO OCI CARD.
1. APPLY ON-LINE at https://passport.gov.in/oci/capchaAction [ OCI Registration (in lieu of valid PIO Card)]
2. Please upload the photograph, signature, copy of the Foreign Passport (one page where the photo, name, date of birth, etc appear), copies of the relevant pages of the PIO card (first page and last page and also any other page where any endorsement has been made ), marriage certificate, if the Indian passport does not have the spouse name endorsed in it and copies of the relevant pages of the passports/OCI/PIO cards of parents. If the marriage was solemnized in Comune, the marriage certificate should first be apostilled from Prefutura and translation in English is mandatory to be uploaded. In case the marriage was solemnized in any other country, the marriage certificate should be attested from the concerned Indian High Commission/Embassy/Consulate.
3. Take the print out of the application form. 6. Copy of Carta D’Identita (self-attested) In the case of minor children of below 18 years, the signature of at least one of the parents should be appended.
7. Copy of the Marriage Certificate – self-attested (if married). If the marriage was solemnized in Comune, the same should be apostilled/attested by Prefuttura and original as well as clear copy of the same along with translation in English should be submitted. Since PIO Cards have only the name of father or spouse and OCI card will be with the names of both the parents and spouse, if married, birth certificate issued from India should be submitted along with the application. In case of foreign nationals, who are holders of PIO cards, by virtue of being married to Indian or Indian origin, they will have to submit birth certificate issued by the respective Government, duly attested by Prefuttura. In case of born in a country other than Italy, the birth certificate should be attested from the concerned Indian High Commission/Embassy/Consulate.
IN THE CASE OF APPLICANTS WHO ARE ALREADY HOLDER OF OCI CARD AND WANT TO GET IT RE-ISSUED DUE TO CHANGE IN PERSONAL/PASSPORT PARTICULARS.

2. Please upload the photograph, signature, copy of the Foreign Passport (one page where the photo, name, date of birth, etc appear), copies of the relevant pages of the existing OCI card (first page and last page ), marriage certificate, if the existing OCI card does have the spouse name endorsed in it and the Indian passport does not have the spouse name endorsed in it. If the marriage was solemnized in Comune, the marriage certificate should first be apostilled from Prefutura and translation in English is mandatory to be uploaded. In case the marriage was solemnized in any other country, the marriage certificate should be attested from the concerned Indian High Commission/Embassy/Consulate.

1. APPLY ON-LINE at https://passport.gov.in/oci/capchaAction and upload the photograph and signature of the applicant.
2. Please upload the photograph, signature, copy of the Foreign Passport (one page where the photo, name, date of birth, etc appear), copy of the Indian passport (first page and last page) or the proof of being an Indian origin, copies of the passports(Italian and Indian)/OCI cards of both the parents.
3. In the case of children, who have never held an Indian passport, please upload the birth certificate (in English) and the copy of the OCI card and/or foreign passport of one of the parents (self-attested by either of the parent).
4. Take the print out of the application form. Please paste one recent colour photograph of the size of 2 inch x 2 inch with light background in the space provided in the first page of the application. The photograph should have a frontal view, showing both the ears. One photograph may be attached with the application form
5. OCI Application form, duly filled in and signed by the applicant (signature should match with that in the passport). 6. In the case of new born children or minor children, both the Both the parents should append their signatures on the second page of the application.
7. Copy of the birth certificate(self-attested by parents) issued by Comune along with its translation in English (in case the child was not issued any Indian passport), duly attested by Prefutura. Please note that one of the parents should be an Indian origin and holder of OCI Card.
8. Copy of DECRETO regarding granting of citizenship, issued by the concerned department(Comune) along with translation in English (SELF-ATTESTED).
9. Copies of the relevant pages of the Foreign passport (self-attested by either of the parent),
10. Copy of Carta D’Identita (self-attested by the applicant or either of the parent) of the child,
11. Copies of the relevant pages of the OCI card/Indian Passport and Foreign passports of both the parents(self-attested)
12. Copies of Carta D’Identita of both the parents (self-attested)
13. Copy of the Permesso Di Soggiorno of the parents(self-attested), if one of them is holder of Indian passport.
14. Application for cancellation of Indian Passport. One photograph to be affixed on the application form.
15. Copies of all the pages of the Indian passport (including blank pages) self-attested on all the pages by either of the parent.
16. In case the Indian passport has already been cancelled earlier, copies of the relevant pages passport where the photo/name/date of birth, etc. appear, page where the names of parents and address appear and the page where the Indian Consulate’s stamp appears with CONSEQUENT ON ACQUIRING OF FOREIGN NATIONALITY, THIS PASSPORT IS CANCELLED AND RETURNED TO THE HOLDER-self-attested by either of the parent.
